[Perspectives] From small beginnings: to build an anti-eugenic future. thelancet.com September, 1921 was unusually hot and New York was sweltering. For the many immigrants who crowded the city's tenements and pavements, one of the few places for relief from the incessant heat was the American Museum of Natural History. That summer the museum presented a new exhibition with rows of human skulls, snapshots of patients in psychiatric institutions, and the preserved brain of a serial killer. It was all terribly macabre. The immigrants among the museum's visitors who read the leaflet distributed at the entrance soon discovered that this exhibition was all about them.

[Correspondence] Where is the next SARS-CoV-2 variant of concern? thelancet.com The COVID-19 pandemic has been characterised by successive waves of new variants of concern sweeping the population. The ultimate source of these variants is not known with certainty, but preliminary evidence suggests at least some have emerged from long-term SARS-CoV-2 infections, such as those observed in immunocompromised patients.1 As a result, it is of the utmost urgency that those with long-term infections should be able to access quality health care and be prioritised for curative therapy because a failure to properly manage these infections poses a risk to the individual and to public health.

Richard Horton (2022-05-21). [Comment] Offline: How to fix pandemic preparedness. thelancet.com Surveillance. Detection. Response. The elements of a robust public health system to prevent the next (inevitable) pandemic are well understood. An early warning system for pneumonias of unknown aetiology is of paramount importance. Debates abound about how to deliver these crucial functions. A pandemic treaty. Revisions to the International Health Regulations. The role of WHO. Financing instruments. Workforce. There is little consensus. The sad truth is that political disagreements will tie these questions up in the mucilaginous bureaucracy that is today's global health.

Susan Bewley (2022-05-21). [Correspondence] HPV vaccination and cervical cancer screening. thelancet.com Screening can cause harm (eg, anxiety, further tests, diagnostic labels, costs, morbidity, and death). Sometimes, a screening programme can bring net benefits when the Wilson and Jungner criteria are applied.1 Screening can detect problems too early, leading to overdiagnosis and overtreatment, resulting in high financial costs, morbidity, and death. Screening healthy people should be considered a medical failure, a second-rate and burdensome approach, and at best should be a temporary, contingent stopgap between the real successes of prevention and cure.

[Perspectives] Deep learning a person's risk of sudden cardiac death. thelancet.com The incidence of sudden cardiac death is increasing globally, despite improvements in the prevention and management of ischaemic heart disease. The cornerstone for primary prevention of sudden cardiac death due to arrhythmia is the deployment of an implantable cardioverter-defibrillator (ICD). Although the survival benefit is indisputable, ICD therapy is resource-intensive and there are risks of complications and morbidity. Accurate risk assessment before device deployment is crucial. But assessment of left ventricular ejection fraction is insensitive.

Peter Sasieni, Alejandra Castanon (2022-05-21). [Correspondence] HPV vaccination and cervical cancer screening — Authors' reply. thelancet.com We agree with Susan Bewley's suggestion that cervical screening should be reviewed in light of human papillomavirus (HPV) vaccination. We have published suggesting that women vaccinated with either the bivalent or quadrivalent vaccine at ages 11—15 years might be screened on just three occasions (approximately at ages 30, 40, and 55 years), and that women vaccinated with the nonavalent vaccine might be screened just twice.1 Our observations2 suggest that even less frequent screening might be appropriate (for those who received the bivalent vaccine) and that herd protection might reduce the need for more int…
